Your child will be enrolled in Summer Camp once the forms listed below are turned in and approved, and tuition and fees have been paid. Please turn in everything together. Completed forms can be scanned and emailed to email@example.com, or dropped off at the school (there is a mail slot on the door for after-hours drop offs). Please contact the Administrative Coordinator if you need a hard copies at firstname.lastname@example.org .
Also, be sure to read the Summer Camp Information sheet for enrolled families. Due to Covid-19, there are significant changes to drop-off/pick-up protocols, etc. which are detailed in this document.

State law requires that your child not be overdue for any of the required immunizations and for us to have a copy of your child’s immunization records on file before he/she can attend.
